Licensing - Request to be removed as Designated Premises Supervisor (DPS)

If you no longer want to be a Designated Premises Supervisor (DPS), you need to give formal notice to us and to the premises licence holder.

Application forms

The forms to make a request to be removed as Designated Premises Supervisor (DPS) are only available from the GOV.UK website which gives guidance on the application procedure.

You can complete the whole application process online from the GOV.UK website, see the following link to application forms and guidance...

PLEASE NOTE:

  • You do not need to pay a fee when you make a request.
  • You must also give formal notice to the premises licence holder that you wish to be removed as Designated Premises Supervisor (DPS)

Once we receive your application we will:

  • Acknowledge it within three working days
  • Notify you of the outcome
